Memory & Dementia Screening

Concerned about you or your loved one's cognitive health or memory? This eight-question screening uses the AD:8 Washington University Dementia Screening test to compare signs of normal aging to potential signs of mild dementia. The screening is not intended for individuals who have a diagnosis of dementia. 

The screening tool can be completed by a family member who knows an individual who may be having cognitive issues.
